The Ryan White Medical Case Management program is available free of charge, to individuals living with HIV /AIDS. Case Management is a client-centered program that links clients with health care and other services as needed.  The primary goal of medical case management is to work with individuals to connect to and engage in their medical care.
At Truman Medical Centers, we have 10 medical case managers at Hospital Hill and one located at Jackson County Health Department.  A medical case manager can assist with a variety of Ryan White service authorizations including access to Ryan White Primary Care and AIDS Drug Assistance Program (ADAP).  Case managers can help you navigate and understand the healthcare system by offering support and by being an active member of your medical team.

To enroll in services you must provide the following documentation:  Proof of HIV-status, proof of income, and proof of residency.  You also must be willing to meet with your case manager at least two times per year and participate in your medical care.

Case managers and clients meet twice a year to evaluate the client’s needs and create an individualized plan of care. Case managers can assist the client in locating and accessing services such as medical care, medications, mental health and substance abuse counseling and care, housing and other community resources.

Ryan White Medical Case Managers are located throughout the Kansas City area in a variety of community settings including local health departments, hospitals, health clinics and community-based organizations.
